In order to protect you, other patients and our staff, we ask that you follow these COVID-19 Office Guidelines:
- Have ALL of the paperwork you received in the mail completed upon arrival.
- Wear a face mask upon entering the office and during your appointment.
- Have no more than ONE individual accompany you to your appointment.
Please note, your temperature will be checked with a forehead thermometer upon checking in. Any patient with a temperature of 100 or higher will not be seen.
We are taking precautions to ensure you do not come in contact with other patients and to clean/ sanitize our spaces between appointments.
